Each additional tenant per unit is charged $1,700 per month.
An additional $200 per month is assessed as each level of service is increased.
Level services is based on a point system from the Functional Assessment & Service Plan.
Assessment tools & Occupancy Agreement are available upon request.

Ancillary Fees

Weekly Laundry Service	$12.50 per week
Transportation Service	$25 each way
Whirlpool Bath	$20

Basic Service Includes:

Three meals per day

All Utilities Except Telephone
Light Housekeeping
Planned Social Activities
Assistance with Activities of Daily Life (ADLs)
Access to Beauty/Barber Shop
Health Assessment/Monitoring
Physical Therapy & Rehabilitation Services
Nurse on-call 24 hours a day
Priority Admission to St. Luke Lutheran Nursing Home, subject to availability

Level II Services

As needs increase, services may increase to Level II, which may include any of the following:

Daily Assistance with Activities of Daily Living
Daily Housekeeping
Daily Medication Management
Monitoring of Medical Needs
Three Meals per Day
Laundry Service
Manageable Incontinence Care
Assistance with Transfers or Ambulation
Physical Therapy & Rehabilitation Services
